How To Prepare

  • Write down your physician’s name
  • Write down the location, include building name and suite number
  • Look at a map of the area or call the office for parking availability
  • Find out whether your insurance is accepted
  • Know your insurance co-pay that is expected at each visit
  • Write down any questions for your doctor or business office

Bring these items with you for your appointment:

  • Completed registration forms
  • All insurance cards and prescription drug coverage cards
  • List of current medications
  • List and contact information for other doctors you have seen

What to expect:

  • An appointment lasting an hour or more. Your doctor will discuss with you your diagnosis and available treatment options
  • To meet your physician and the healthcare team
  • Additional tests performed or ordered
  • Additional information on the treatment you and your doctor decide will be best for you
