Insurance
We recognize insurance contracts can be confusing and the cost of health care overwhelming. For this reason, we have a staff of insurance specialists and financial coordinators willing and able to help you with this process. In turn, we ask you to be familiar with the major details of your individual policy/policies including prior authorizations, co-pays and deductibles.
Keeping us informed of your most current insurance information is the first step. Please bring your insurance cards to each visit. Our registration specialist will ask for your card when you check in and make a copy for our files. We use this information to verify your coverage and/or implement assistance where indicated. In addition, our registration specialist will collect your co-pay and/or receive any payments you wish to make when you check in.
Insurance Authorization and Billing
We have relationships with many insurance groups. This means we are able to bill them directly for services you receive here. In the event that you need chemotherapy, we will contact your insurance company before you begin (a process called “prior authorization”) to confirm you have coverage for such therapy. This process can often take 24 hours to complete. If we discover that you do not have coverage, be assured that we will make every effort to investigate potential resource options for you (visit Financial Assistance for some options). This process will include a meeting with one of our staff social workers.
Statements are sent out on a monthly basis. Any charges that remain after your insurance has paid their portion are your responsibility. For your convenience we accept cash, check, money order, debit cards, MasterCard, Visa and Discover. In the event that these are not options, we will set up a contracted payment plan.

Insurance Forms
We have staff designated to assist you with completion of insurance forms directly related to your cancer condition. Examples of forms include: Social Security disability forms, long and short-term disability forms, FMLA forms, etc. However, there is a fee for completion of forms:
$10 for FMLA forms
$20 for all other forms
The estimated time for completion is three weeks, and the fees must be paid prior to the patient receiving completed forms. Forms may be dropped off, mailed, or faxed to the office. If a patient is unable to pay for form completion, a financial coordinator is available to assist you with potential options.
