Referring Providers

Care sings loudest when we all play together. By collaborating with a patient’s referring physician, our expert  physicians, nurses, therapists, technologists and other health care professionals provide the highest quality care for our patients—regardless of how they came to be at our door. Our locations across West Michigan are designed to make referrals to CHCWM as easy and convenient as possible for you and your patients.

Our staff wants to become familiar with you, your practice and what you are looking for in a referring provider relationship. With your help, we want to:

  • promote positive referring provider relationships
  • encourage effective communication
  • provide forums for education and learning
  • provide information on what is happening at CHCWM

As a physician, your call will be received by a CHCWM Call Center representative. Our representatives can promptly schedule a patient appointment, reach a consulting physician and provide help with any needed arrangements or information about Cancer & Hematology Centers of Western Michigan.

Founded in 2009 as a nonprofit corporation by leading health systems in Michigan, GLHC set out to advance the delivery and coordination of high-quality, efficient patient-focused health care across the state. Combining expertise in information technology and clinical data exchange, Great Lakes Health Connect has built the largest, most trusted HIE network in Michigan.
